申请美国护照后如何更改寄送地址

读者 Woo Wo 之前在群里问，初次申请美国护照，如何更改 delivery address, 后来给国务院打电话，经过无数次拨打才接通，客服提供了一个表格，填好之后发送回去即可。这个表格在网上不好找，感谢 Woo Wo 分享！点击这里下载。

表格是 “US Department of State Passport Services” 的 “Change of Address Request Form” 。如果你申请护照或护照卡，在等待的过程中搬家了，需要改地址，请填写此表并寄回给 CA-PPT-HQCallCenter@state.gov 即可。如果是多人申请，每人需要单独提交。
